TEMPEST RAPID REPORT FORMAT

TEMPEST RAPID 0-V-00-000
PART I

1. Narrative statement concerning the nature of the disaster, extent of
damage, the estimated duration, and location to include, city, county, and
State where appropriate. Also include FDAA disaster identification num-
ber if assigned.

2. Degree to which civil authorities have committed their resources.
8. Source of request for military assistance.

4. Name, location and telephone number of the Federal Coordinating
Officer (if one appointed).

5. Name, location and telephone number of the DOD Military Representa-
tive.

6. Casualties and fatalities of both military and civilian personnel. Re-
port by category for both reporting period and cumulative total similar
to the format discussed in paragraph 1-8b6 (3) and table 1-1.

7. Army resources committed.

a. Active Army.

PART II

(1) Narrative statement concerning participation by Army forces. Narrative to be in sufficient detail to identify significant support operations conducted. (Sample narrative-"During the period of 040600Z to 050600Z Mar 71, Army support consisted of feeding 1325 meals; flying 82 helicopter sorties rescuing 41 stranded residents, transporting 35 doctors and six disaster officials; re-establishing communications; and providing additional tentage and medicine to ARC. Cumulative total to date is 3230 meals and 101 helicopter sorties (54 persons rescued and 61 passengers delivered).")

(2) Personnel committed-Identify military and civilian personnel by installation and activity for both reporting period and peak commitment. (3) Equipment committed-Identify by installation for both reporting period and peak commitment.
